Ph. D. Application Checklist
The following is a list of items required for your application. Use this checklist to help you in gathering all the necessary documents.
Application
Electronic graduate application.
Application fee of $50. Pay online with credit card.
Academic Documents
Two (2) official academic transcripts directly from all colleges/universities attended, showing all academic coursework and degree/s awarded.
Certified copy of Bachelor's Degree (diploma) awarded, if degree conferred is not posted on final transcript.
Three (3) recommendations for admission. Recommendation forms in PDF format may be printed from the Website.
A Statement of Purpose. A 400-500 word essay summarizing your academic background, research, and professional experience if relevant, educational goals and objectives, and detailing your specific area/s of interest in graduate study. And
Official GRE score report from ETS. IUPUI institution code: 1325.
Note: U.S. Permanent Residents and Foreign-born Naturalized U.S. Citizens: If you are either a U.S. permanent resident or a foreign-born naturalzed U.S. Citizen and have completed your Bachelor's (undergraduate) degree from a university or college outside the United States of America, official TOEFL test score report from ETS is required for your application. In addition, include with your application two (2) photocopies of your U.S. permanent resident card ("green card") or your U.S. Passport.

3. Compose or attach the Statement of Purpose in the electronic application.
