The following items are required to request for a Purdue University graduate-level course to be offered at IUPUI (contact Valerie Lim Diemer wvlim@iupui.edu for assistance, if needed): 1. Purdue University "Graduate Form 40G" available at this website: a. If the course is a new course, include the accompanying “Cover Memo to Purdue University Graduate Council” which is a memo-form following the Form 40G. The cover memo is not needed, if the course is an existing Purdue course. b. Prepare PU Graduate Form 40G using the exact course details (course number and title, abbreviated title, credit hours, course description, prerequisites, etc.) provided at the Purdue University “Course Information” website for the West Lafayette campus: http://www.courses.purdue.edu/cgi-bin/relay.exe/query?qid=courseCatalogSubjectList 2. Each course request (whether an existing or a new course) must be accompanied by supporting documentation/s , no more than 4 pages , consisting of the following information: I. Justification/rationale for the course: Justification or rationale for the course Level of the proposed course and anticipated student enrollment Course instructor
II. Course information and Syllabus: Course number and title Credit hours, fixed or variable Minutes and number of meetings per week, if determined Objective/s of the course Prerequisites, if any Text/s and supplemental readings Course outline Evaluation and grading scale Cheating and plagiarism Americans with Disabilities Act Refer to or use the attached “ Suggested Course Syllabus Format ” template provided by the IUPUI Graduate Office in preparing the documentation. 3. Prepare the Indiana University " New Course Request " multi-carbon form. The Graduate Programs Office has a supply of this IU forms. This IU multi-carbon form must be typed with a manual typewriter and is not available online. Use the exact course details provided at the Purdue University Course Information website in preparing this IU form. 4. Submit all course requests to Valerie Lim Diemer in the Graduate Programs Office to begin the routing and approval process. 5. Note: The length of time for a course request to be fully approved and ready for scheduling is approximately 15 months.
