If you are receiving more than one certificate, you must complete a separate Certificate Application for Graduation form for EACH certificate you are receiving.
When completing this application, be sure to print all information.

D. Student's 10-digit university ID number
E. Permanent home address
F. Home phone number and work phone number
G. IUPUI email address
H. Date (month and year) of graduation
