Deadlines for submitting Undergraduate Applications:
May graduation,
February 1.
August graduation,
April 15.
December graduation, September 15.
This Undergraduate Application for Graduation form must be returned to ET215.
If you are receiving more than one degree, you must complete a separate Undergraduate Application for Graduation form for EACH degree you are receiving.
When completing this application, be sure to print all information.
A. Date the student completes this application.
B. The department/school from which the student expects the degree
C. Student's full name
D. Student's 10-digit university ID number
E. Permanent home address
F. Home phone number and work phone number
G. IUPUI email address
H. Date (month and year) of graduation
I. Select one or more of the undergraduate degree options for which you are applying. If you will also be receiving a certificate, you must complete a separate graduation application form to receive your certificate.
Do not complete the backside of this application form.
